We are pleased to welcome Tim Fitzpatrick Advance Care Planning Consultant.
Tim joins the team at Highton Clinic from a background in nursing, initially in general and surgical nursing, then palliative care, and finally in advance care planning.
Advance care planning gives you the opportunity to document your preferences for treatments you would want if you became ill, should you suddenly become unable to express these at the time; for instance, if you were unconscious after an accident, or unable to speak following a medical event, such as a stroke. Or, if you have a chronic condition, and feel your health is changing, an Advance Care Directive is an opportunity to address this.
It allows you also, to have someone you trust, and who knows you well, to speak on your behalf, in such a situation. This is known as a Medical Treatment Decision Maker, (previously referred to as a Medical Power of Attorney). If you have appointed someone in this capacity, it is important to let them know your thoughts on such important issues. This is where Advance Care Planning can help.
Tim is available here at Highton Clinic to help you with this, at no cost to you.
He can help you prepare and complete these documents, so that you can express your thoughts on these matters clearly, and well ahead of any medical emergency, should it arise. It allows you to let your doctor and your family know how you feel, and what you want.
